How Long Does a Hip Replacement Last? The Answer Just Changed

Of all the questions I get asked about hip replacement, this one comes up the most: "How long will it last?"

And I understand why. Nobody wants to go through a big operation twice. Many of my patients watched a parent or a neighbor get a hip in the 1980s or '90s, wear it out, and need a second surgery — a "revision" — fifteen or twenty years later. That memory shapes how people think about the decision today. Some put off a surgery they need for years, trying to be "old enough" that the implant will outlive them.

Here's the good news: the implant your neighbor got in 1995 is not the implant we put in today. And now we finally have the numbers to prove it.

The old numbers

For years, the best answer we had came from a large 2019 review in The Lancet — one of the world's top medical journals. Looking back at hip replacements done decades ago, it found that about 58% were still in place at 25 years. Not bad — but it meant telling patients that roughly four in ten hips would need a re-do within a quarter century. Those numbers, though, mostly described older implants with older materials. They were a rearview mirror.

The new numbers

In February 2026, The Lancet published the largest study ever done on modern hip replacements: 1.9 million hips, combining 29 clinical studies with national joint registries from eight countries — the databases that track nearly every joint replacement done in places like Australia, the UK, and Scandinavia.

The results, for implants using today's bearing materials:

  • 93.6% still in place at 20 years
  • A projected 92.8% at 25 years
  • A projected 92.1% at 30 years

Read that again. The old data said about 58 out of 100 hips made it to 25 years. The new data says about 93 out of 100. For patients, that is a genuinely different conversation.

What changed?

Mostly one thing: the plastic got dramatically better.

A hip replacement is a ball moving inside a socket, and the socket is lined with a plastic bearing. In the older generation of implants, that plastic slowly wore down — and the microscopic wear particles irritated the bone, loosening the implant over time. Wear was the clock ticking inside every hip.

Around twenty years ago, engineers learned to treat that plastic with radiation to cross-link its molecules — think of it as weaving the fibers into a much tighter fabric. This "highly cross-linked polyethylene," along with modern ceramic ball heads, cut the wear rate to a small fraction of what it was. Slower wear means the clock ticks much, much slower. The new Lancet study is the proof, at scale, that the engineering worked.

What this means if you're putting surgery off

I still believe most arthritic joints should try everything reasonable before surgery — that hasn't changed, and it never will in my practice. But if your hip is truly failing you, the old fear — "I'm too young; I'll just wear it out and face a terrible second surgery" — carries a lot less weight than it used to.

A 60-year-old getting a modern hip replacement today has a very good chance of never needing another hip operation in their lifetime. That was not something surgeons could honestly say twenty years ago. Now we can.

The honest fine print

I promised you straight answers, so here are the caveats:

  • The 25- and 30-year figures are projections. The modern materials have about 20 years of real-world tracking; the study used careful statistical modeling to extend the curve. The 20-year number — 93.6% — is measured, not modeled.
  • "Still in place" is the measure, not perfection. These studies count whether the implant needed a re-do operation. The great majority of surviving hips work well, but survivorship and satisfaction aren't identical.
  • You are not a statistic. Infection, falls, bone quality, weight, and activity all still matter, and younger, highly active patients do revise somewhat more often. Your personal odds are a conversation, not a table in a journal.

The bottom line

Hip replacement was already one of the most successful operations in all of medicine. The newest and largest evidence we have says the modern version is also one of the most durable — with more than 9 in 10 lasting 30 years.
